After more than seven years of dedicated service to our patients at El Dorado Family Medicine, we have made the decision to transition to a new opportunity, which we believe will enhance the quality of care available to you, your family and the entire Santa Fe community.
As of October 7, 2024 El Dorado Family Medicine will close and merge with Nexus Health, a new multispecialty practice committed to providing high-quality care at an affordable cost. This merger will provide you access to a broader range of healthcare services and specialties, while also ensuring a more comprehensive approach to care under one roof.
We are excited about this new opportunity and what it means for our patients. With that in mind, the transition of your care is of utmost importance. To prepare for this transition, please find key information below regarding upcoming appointments, medical records and appropriate contact information for Nexus Health.
- Scheduled Appointments
For patients with appointments scheduled on or after October 7, these will now take place at Nexus Health, located at 2085 S Pacheco St. Santa Fe, 87505. Our scheduling team will contact you to confirm your appointment and address any questions you have about the new location. - Continuity of Care
All medical records will remain in the care of El Dorado Family Medicine. As we transition to Nexus Health, your records will remain securely in our system. If you have any questions or concerns. Please don’t hesitate to contact us. - New Location and Contact Information
